Implementing Chronic Kidney Disease Guidelines in General Practice
This presentation will focus on the implementation of education to general practice nursing staff and physicians by creating staged education, the introduction and adoption of standardised processes within general practice to create new screening pathway to ensure all patients are screened for chronic kidney disease using the 2012 CKD General Practice Guidelines, and the early identification of patients at risk of chronic kidney disease and prioritisation of patients requiring further investigations, monitoring and referral.
