Investing in Prevention Primary Health Care Nurses Drive Cardiovascular Disease Assessment- Amanda Jennings & Kerry Hollier
This pilot project demonstrated the importance of using an absolute risk approach to assess cardiovascular risk in eligible patients and how general practice nurses play a vital role in implementing change within their practice.
Supported by the Heart Foundation, nurses led changes to systems, to ensure appropriate patients were identified, assessments undertaken and recorded and relevant information provided to patients.
