Bone Health Consults - Andrea Hunter & Kim Weeks
The purpose of the Bone Health Consult is to identify, screen, educate and treat patients at risk of osteoporosis, and in the long term to prevent fractures and complications including early death.
This new nurse-led project enhances nurse leadership and team-based care. It targets early intervention of complex care and care coordination for identified patients. This new and innovative approach also empowers patients to take some control over the ageing process and avoid some of the health complications of osteoporosis.
The process is sustainable and could be used to target other chronic conditions. In targeting pre-aged care demographics Bone Health Consult hope to reduce the need for eventual admission by keeping them safe at home.
