Nurse Navigator North West Hospital & Health Service - Anne Neilson, Kea Apineru & Alexandra Rungie
The North West Nurse Navigator program delivers a contemporary nursing model of care championed by Queensland Health’s Office of the Chief Nursing and Midwifery Officer. This initiative utilises highly experienced nurses to assist patients with complex needs, to navigate fragmented healthcare systems. In the context of health policy this nurse-led model focuses on prevention and primary health rather than acute-based care.
The North West Queensland region has a high indigenous and socioeconomic disadvantaged population, poor health status, and covers a geographically challenged area with all regions classified as remote or very remote Australia. These factors have resulted in the development and implementation of a unique co-ordinated model which meets the needs of remote Queenslanders. The program has included risk stratification tools, innovative triaging systems and complex case reviews to advocate, coordinate care and educate patient and families to improve health literacy and empower them to reach their goals.
