Using data to inform quality improvement activity in general practice
The presentation by Beverly Korn and Christine Ash will detail the Gold Coast PHN's tiered approach to practice support that includes working with the general practice team including nurses to analyse demographic and clinical data to support and promote quality improvement activity. The activity promotes the importance of reviewing, monitoring and using data to inform and guide best practice management of patients with chronic diseases. The chronic disease focus areas include diabetes, chronic obstructive pulmonary disease, chronic kidney disease and coronary heart disease.
