A Collaborative Model for Enhanced Elder Care
This presentation aims to take you on a journey of cross-sectoral partners who all want to improve the care of elderly people and ensure that the best care possible is provided in the most appropriate setting for their needs. With this in mind the CEDRiC Collaboration (Care Coordination through Emergency Department, Residential Aged Care and Primary Health Collaboration) set forth to obtain formal research to validate the intercepting HIPS(Health Intervention Project for Seniors) and GEDI (Geriatric Emergency Department Intervention) projects.
This particular presentation will focus on the HIPS portion of the project which aims to use an evolving Nurse Practitioner Candidate to improve access to care for elders living in Residential Care. It is hoped that this model will prove to be sustainable and replicable across the aged care sector in the future.
