As the proportion of elderly Australians increase so does the demand for aged care services.
Navigating and accessing the aged care system can be difficult for the majority of individuals, as they are unaware of where to start and the options available to them. The Aged Care Advice Hub using the professional expertise of an Industry Liaison Clinician (ILC) is able to provide local, up-to-date, non-biased information about service availability and suitability for the individual.
Using a collaborative model the ILC works with general practice to identify potential patients that may benefit from the free service.
