16 April 2018. Australia’s inaugural Advance Care Planning Week begins on 16 April, encouraging people of all ages to prepare their personal end of life care plans, to give certainty to themselves and their families. Department of Health Media release.
This session is for primary care nurses who participate in care planning and treatment of patients with chronic or life-limiting conditions, enabling you to learn about the principles and practice of advance care planning.
This course will strengthen your understanding of how to support healthy ageing and encourage you to further expand the activities you undertake to optimise health as people age.
